If you’re gambling too much money, spending too much time at the casino, or feeling unhappy with how gambling is affecting your life, self-exclusion can help.

When you sign up for statewide self-exclusion, you are automatically added to ban lists at casinos across Oklahoma.

Electronically

We now offer the self-exclusion form online. You can submit your form online, and OAPGG staff will schedule a brief online notary session to confirm your identity.

By Mail

Print and complete the form, get it notarized, and mail it to the OAPGG office along with the additional required documents.

In-Person

Come to the OAPGG office and we will help you complete the form in-person. Please call ahead to make sure that someone will be available to assist you: (405) 801-3329.

How to Complete Your Self-Exclusion Form

This may feel like a big step, but remember that you are not alone.
These instructions will walk you through everything one step at a time.

Gather What You’ll Need

Before you begin, please gather the following items:

  • Your driver’s license

  • A recent photo of yourself (not your driver’s license photo; selfies are OK)

Fill out the Form

Go through each section on the form and fill in your information. You’ll need to:

  • Write your full legal name

  • List any nicknames or aliases

  • Provide your birth date, address, and contact info

  • Fill out your physical description (height, weight, hair, eyes, etc.)

Take your time. It’s okay to pause and come back to it.

Choose Your Timeframe

You'll see boxes for 1 year, 3 years, 5 years, or 10 years. Check the timeframe that feels right for you.

Please note that once you are added to the self-exclusion list, the timeframe cannot be changed.

Initial Each Statement

There are several statements that explain:

  • What it means to be self-excluded

  • What casinos will do if you return to their property

  • What your responsibilities are

After reading each one, write your initials on the line.

Notarize Your Form

If you are completing the form online, we will verify your identity through a brief meeting on Notary Hub.

If you are mailing the form, bring the form, photo, and ID to a Notary Public.

  • You must sign the form in front of them

  • The notary will also fill out part of the form and stamp it

Make sure the date you sign is the same as the notary’s date.

You can also bring these items to be notarized at the OAPGG office in Norman (501 E Alameda, Suite E, Norman, OK 73071). Please call ahead to verify that this service is available (405-801-3329).

Attach the Required Documents

Attach the following documents to your application:

  • A copy of your driver’s license

  • A separate, recent photo of yourself (selfies are okay)

While most tribes participate in the self-exclusion program, not all tribes currently participate. Tribal nations may choose to opt in at any time. As a result, the tribal participation list is ever-changing.
